Participate in an interdisciplinary team to better serve the Center population. Provide therapeutic intervention to help residents cope with social and psychological aspects of illnesses, impairments, or disabilities, and their feelings about institutionalization and separation from family, community, physical and emotional loss. Maintain written documentation in the resident medical record per the company’s policy. Perform an initial evaluation/assessment of each resident’s psychosocial needs; document at least quarterly. Short‑term residents require ongoing written documentation. Develop the social service component of the resident’s plan of care identifying specific problems, goals and approaches. Participate in and coordinate interdisciplinary care‑planning conferences at admission and required intervals to assist in developing and revising the overall plan of care. Ensure and document that residents and/or families have input into the care‑planning process. Act as a liaison between residents, families and outside agencies and the Center Administrator to ensure resident rights are maintained and upheld. Conduct family meetings as needed (e.g., new admission group meetings, support group, family and friends council). Facilitate discharge planning, including the development of an organized discharge plan for all residents. Maintain the safety of all Center customers to minimize potential for fire and accidents. Coordinate admissions to the facility by generating referrals. Develop and maintain relationships with area referral sources. Verify insurance coverage through computer systems or verbal authorization. Conduct tours of the facility for outside interested parties. Position Requirements
Possess a CSW or LSW license in the state of operations. 1 year of Social Work experience. Long‑term care/skilled nursing experience preferred. About Us
