Reports To:
Care Team Manager. At Reema Health, we believe that a personalized, community-based approach to care makes it easier for anyone to access the things they need to improve their lives. We seek a experienced and self-motivated Community Guide who has worked with individuals living with health challenges and social issues, and understands the unique needs of this community. This energetic and adaptable Community Guide is part of a start-up that aims to transform how health care and social care work together. The Guide builds genuine, positive relationships with members to improve health and wellbeing, using an integrated technology platform to call and text members, schedule community visits, and document all interactions. The Guide manages the full lifecycle of their caseload, including initial outreach, ongoing engagement to connect members with health care and community resources, and graduation from the program. A primary strength for this role is excellent verbal and written communication, the ability to build strong meaningful relationships, problem solving, and comfort with the evolving nature of a startup. This position requires significant time in the field, directly interacting with members who may have experienced trauma and face barriers related to mental, physical, and chemical health. Responsibilities
Engage and work with clients from diverse ethnic backgrounds, focusing on those with complex social needs and chronic health conditions. Reach out to members and share about Reema Health to enroll them in the care model. Assess member needs and support systems, including social determinants of health, to help create a member-centered care plan. Establish trusting relationships with members, provide meaningful support, and follow through in a timely and appropriate manner to achieve wellbeing goals. Coordinate referrals and schedule/track attendance with behavioral health, primary care, and other service types for members. Manage a caseload of 60–100 members and provide coverage for other Community Guides during periods of absence or increased workload. Organize and prioritize workload to achieve monthly goals and outcomes; independently manage time and tasks. Demonstrate active listening, empathy, and respect for member vulnerability; maintain professional boundaries and seek consultation with the multidisciplinary team when needed. Visit members in person as appropriate, which may include homes, communities, hospitals, or other treatment settings. Use text, phone, email, and face-to-face interactions to match each member’s communication preferences. Identify and connect members to community and health resources, including transporting members to access services when needed. Prepare members for services ending by promoting self-sufficiency and independence. Document all work thoroughly and in a timely manner using the Reema App, including keeping care plans updated; maintain confidentiality of information per HIPAA. Education, Experience and Skills
Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences or related field; related experience may substitute for degree requirements Three or more years of direct experience with the population or community served, with knowledge of community Commitment to helping individuals with complex health needs and belief in recovery Experience with crisis planning, case management, boundaries, and personal safety in community work Strong critical thinking, time management, strategic planning, and multitasking abilities Proven ability to build and maintain relationships, initiate conversations, and create connections Ability to develop, adapt, and execute outreach and care plans Proficiency with technology, including Microsoft Word, Outlook, Excel, and video conferencing; G-Suite preferred Qualifications & Position Requirements
