Overview
Reema Health, a start‑up transforming behavioral and social care, is seeking a self‑motivated Behavioral Health Community Guide to work with individuals living with serious mental illness and substance use disorder. The Guide will collaborate with a multidisciplinary care team to plan and coordinate services for members, building genuine relationships and improving member health and wellbeing.
Work Schedule: Monday–Friday with occasional evenings and/or weekends Vehicle Required: Yes Amount of Travel Required: 25‑50% within a 30‑mi community service area Reports To: Care Team Manager
Responsibilities
Outreach and engagement into care – engage clients from diverse backgrounds, enroll them in Reema Health’s care model.
Manage a caseload of 60‑100 members, providing coverage for other Community Guides when needed.
Assess member needs and social determinants of health to create patient‑centered care plans.
Guide members through enrollment in behavioral and mental health services, collaborating with clinicians and establishing trusting relationships.
Coordinate referrals and support appointment scheduling for behavioral health, primary care, and other services.
Organize and prioritize workload to achieve monthly goals, using effective time and task management skills.
Maintain professional boundaries and confidentiality, seeking consultation on complex situations.
Visit members in person when appropriate, utilizing phone, email, text, or face‑to‑face interactions as needed.
Identify and connect members to community and health resources, including transportation assistance.
Prepare members for service transitions, encouraging self‑sufficiency and independence.
Documentation and Representation
Use the Reema App and EHR to document all interactions and maintain up‑to‑date care plans.
Ensure compliance with HIPAA and confidentiality of protected health information.
Represent Reema positively, working independently and collaboratively in a fast‑paced environment.
Build relationships with community resources and partners, contributing insights to the multidisciplinary team.
Education, Experience and Skills
Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences or related field (experience can substitute for years of education).
3+ years of direct experience with the population served and knowledge of the community.
Experience with crisis planning, case management, boundaries and personal safety in community work.
Proficiencies in critical thinking, time management, strategizing and multitasking.
Strong relational skills, initial conversation building and connection creation.
Tech proficiency: Microsoft Office, Video Conferencing; G‑Suite preferred.
Qualifications & Position Requirements
18 years of age or older.
Valid driver’s license, vehicle insurance and satisfactory driving record.
Pass pre‑employment drug screening and background check.
Compliance with federal, state and local privacy regulations (HIPAA).
Use of technology‑based tools and systems daily.
Occasional lifting and carrying up to 25 pounds.
