About Path Founded in 1984, PATH (People Assisting The Homeless) pioneers bold approaches to assist people experiencing homelessness. We operate throughout California, linking clients to services such as homelessness prevention, street outreach, employment preparation, individualized case management, supportive services, interim and permanent housing.

About The Job The Case Manager assists clients to break the cycle of homelessness through a “whatever it takes” approach. This role supports clients in all phases of homelessness—on the street, interim housing, transitional housing, and permanent housing—by helping them develop and maintain individualized supportive services.

Conduct intake and individualized needs assessment and collaborate to develop an Individualized Service Plan (ISP) that addresses housing goals, income, medical, mental health, substance use, financial, vocational, and social support needs.

Provide referrals, linkages, information, and support to help participants achieve their ISP goals.

Complete program‑specific assessments such as VI‑SPDAT.

Evaluate strengths and challenges, conduct a 90‑day review with participants after initial intake.

Preferred Qualifications

Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations.

Minimum Qualifications

All levels of education and experience are welcome.

Mandatory Requirements

Have employment eligibility verification.

Have or be willing to obtain CPR/First Aid training.

Successfully complete the following as a condition of hire:

Tuberculosis Test

Background Screening

Drug Test

Driving

Have reliable transportation.

A valid driver’s license.

Proof of insurance and ownership for personal vehicles used during work duties.

The ability to qualify for PATH’s insurance coverage.

Employment Eligibility Applicants must be authorized to work in the U.S.; we do not sponsor visas.

California‑Based Work Expectations All work is performed within California; remote work is not available. Hybrid schedules may involve multiple designated worksites, but no work is conducted from home.
