Salary: $27.75 to $33.83 per hour
Pay Frequency: 24 Pay Periods Per Year

About PATH Since 1984, PATH (People Assisting The Homeless) has pioneered bold, effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement, individualized case management, supportive services, interim housing, and permanent supportive housing.
About the Job As part of the Interim Housing Services team, the Case Manager assists clients in breaking the cycle of homelessness through a “whatever it takes” approach. The position supports clients in all phases of homelessness—including street, interim housing, transitional housing, and permanent housing—by helping them access and maintain necessary services such as healthcare, social services, interpersonal support systems, and coordinated entry.
Position Responsibilities
Conduct intake and individualized needs assessment for all participants; collaborate to develop Individualized Service Plans (ISP) addressing housing goals, income maintenance, and other personal goals including medical, mental health, substance use, financial resources, vocational, and social support needs.
Provide referrals, linkages, information, and support for participants to achieve their ISP goals.
Complete program‑specific assessments (e.g., VI‑SPDAT).
Evaluate strengths and challenges; conduct a 90‑day review with participants after initial intake.
Maintain documentation standards and complete case file notes in a timely manner; ensure a complete, accurate, current participant file with all required documents.
Maintain confidentiality of participant files per HIPAA and applicable guidelines.
Develop therapeutic and/or professional relationships with clients and systems/partners to enhance engagement and retention.
Provide crisis intervention and risk assessment services focused on safety and empowerment of clients with chronic trauma.
Use motivational interviewing and trauma‑informed care in service delivery.
Maintain current, thorough knowledge of community resources and provide comprehensive, wrap‑around services.
Attend and engage in all required training sessions assigned by the company and apply learned skills to daily tasks.
Attend and participate in all required staff meetings and events on and off‑site (when applicable).
Demonstrate a commitment to diversity, equity, and inclusion in all interactions and service delivery.
Perform all other duties as assigned or required.

Preferred Qualifications
Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.
Minimum Qualifications
All levels of education and experience are welcome.
Mandatory Requirements
Employment eligibility verification.
CPR/First Aid training or willingness to obtain.
Successfully complete the following as a condition of hire:
Tuberculosis Test
Background Screening
Drug Test
DRIVING
Have reliable transportation.
A valid driver’s license.
Proof of insurance and ownership for personal vehicles used during work duties.
Ability to qualify for PATH’s insurance coverage.
EMPLOYMENT ELIGIBILITY Applicants must be authorized to work for any employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa. PATH does not employ individuals currently receiving services from PATH or who received services within the past two years.
CALIFORNIA-BASED WORK EXPECTATIONS All regular and customary work must be performed within California. Approval for work outside of the office does not establish approval for work outside of California. PATH does not offer fully remote positions; some roles may include a hybrid schedule and approved off‑site locations.
