2-1-1 Community Navigation Specialist (Maintenance) Under the direction of the 2-1-1 Program Supervisor, the Community Navigation Specialist provides confidential, appropriate, unbiased navigation services based on the Community Hub/Pathways model to a specified target population. This position provides health and social service information, referral, and advocacy by telephone, face‑to‑face, email, mail; performs preliminary screening for eligibility for public and community services; identifies barriers and actively works to reduce or eliminate barriers, assists callers with access issues; tracks gaps in services, and ensures that accurate information is available to all interested parties.
Essential Functions
Assist clients with determining and connecting with community resources and supports and overcoming barriers affecting their Social Determinants of Health (SDOH).
Screens clients for social needs, accurately records transaction information and insurance billing information, refers to appropriate resources, and advocates for individuals having trouble or lacking ability needed to make effective contact with their medical home, agencies, programs and/or services.
Coordinates face‑to‑face client support with appropriate community resources as needed.
Attends community resources appointments with clients as needed. These may require travel and generally occur in Jackson County but travel within the State of Michigan may be necessary.
Tracks gaps in service; capture changing information on resources and eligibility requirements to ensure accurate information is available to all agencies, callers, and assigned staff.
Works closely with applicable community partners, staff at LifeWays, and health‑related agencies and programs to ensure strong coordination and effective service.
Assists gathering program outcome data via phone calls and document results per protocol.
Researches answers to new questions and requests for unknown services.
Provides follow‑up with clients to determine if additional assistance is required and/or desired and if social need has been resolved.
Demonstrates Customer Service skills and ability to work effectively with people of a diverse ethnic, socio/economic background and medical condition with a non‑judgmental, respectful, and friendly demeanor.
Stays informed on broad social issues and changes that impact human services in Michigan with emphasis on Medicaid and social determinants of health.
Effectively represents 2-1-1 and LifeWays to other agencies and groups in person, via email and on the telephone.
Responsible for adherence to all Commission on Accreditation of Rehabilitation Facilities (CARF) and Michigan Department of Health and Human Services (MDHHS) standards. Follow standards set by Alliance of Information & Referral Systems (AIRS).
