Activities Leader – Ten Broeck Center for Rehabilitation & Nursing
Location: 1 Commons Drive, Lake Katrine, NY 12449.
Pay: $16.00 / hour
Shift: Full‑time day shift with alternating weekends.

Essential Functions
Assists in providing recreational and other activities per individualized plans.
Leads or facilitates activity groups such as religious services, physical & cognitive games, arts and crafts, gardening, and grooming.
Communicates with residents who have visual, speech, auditory, cognitive, physical, or emotional impairments.
Completes activity sections of MDS forms on schedule.
Assesses residents’ social and emotional needs with the Director and sets appropriate goals.
Organizes activity materials and decorates the facility with seasonal decorations.
Provides input on monthly activity calendars and displays them on units.
Works with the interdisciplinary team, attending weekly care plan meetings.
Reports all mistreatment, neglect, or abuse cases, including injuries of unknown source.
Assists with transportation to and from activities when needed.
Supports residents in individual activities, including equipment setup and positioning assistance.
Informs residents of available activities when staff are unavailable.
Ensures residents’ safety at all times.
Performs other duties as requested.
Physical Requirements
Transports residents to in‑house activities and community programs.
Moves throughout the building as required.
Pushes up to 225 lbs. in a wheelchair, recliner, or geriatric chair.
Leads exercise classes that involve pushing, bending, and stretching.
