Job Description
The Care Manager (Caregiver) assists residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. This position is a non-exempt, hourly position reporting to the Health & Wellness Director.
Salary Range: $21.00 - $23.00 Hourly
Responsibilities and Duties
Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
Respects the needs of team members and residents by adhering to schedule and documenting attendance properly.
Maintain the safety, comfort, dignity, and confidentiality of all residents
Encourages and escorts (when needed) residents to meals and activities
Ensures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts
Document all services provided in ALIS EHR for your assigned residents during your shift
Monitors and reports any change of condition in a resident immediately to your supervisor
Appropriately utilizes and keeps electronic documentation “devices” safe and secure within the community; efficiently documents care/services provided during the shift.
Participates in and attends all in-service trainings as scheduled
Ensures compliance with all Federal, State, and local regulations, as well as community policies and procedures
Provides basic housekeeping and/or laundry services when needed
May be asked to obtain weights and vitals of residents and document same per RN request
Other duties as assigned by supervisor, RN, and/or ED
Qualifications
High School Diploma or GED. At least 1 year resident care experience required.
Certified Nursing Assistant or Certified Home Health Aid
Knowledge and experience in working with the elderly population, preferred dementia experience
Physical Abilities
While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
Additional Information Valuing a connection to the community, we are creating a pedestrian-friendly front plaza which will offer art fairs, farmers markets, and festivals. Monarch believes its buildings serve residents, their families, and the Monarch team members. Therefore, they have created a team member and resident co-working space; art, creative, and performance spaces; and the WELL center (fitness, yoga, spa, and a fully-stocked juice bar) which can be used by all.
