We are seeking a licensed Psychiatric Mental Health Nurse Practitioner to provide ongoing psychiatric services within residential and post-acute care environments. This role is well-suited for a
mission-driven clinician
who is passionate about improving access to mental health care for an
underserved adult and geriatric population . The position offers a consistent schedule, meaningful patient relationships, and the ability to practice independently in a non-acute setting. Requirements
Perform comprehensive psychiatric evaluations and deliver longitudinal follow-up care Oversee psychotropic medication management and monitor clinical outcomes Develop and refine individualized treatment plans aligned with patient needs Partner closely with nursing staff, facility leadership, and interdisciplinary teams Communicate clearly and compassionately with patients and involved family members Deliver psychiatric care using patient-centered and trauma-informed principles Maintain accurate, timely documentation in accordance with regulatory standards Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) Strong clinical judgment, communication, and interpersonal skills Ability to function independently while contributing to a collaborative care model Preferred Experience
