Physical Requirements : The physical requirements for this position include regularly sitting, standing, walking, with some pushing, pulling, and/or lifting objects weighing up to 10lbs and occasional pushing, pulling, and/or lifting objects up to 50lbs.

Shift : Monday Friday, Day Shift; primarily 8:00 am - 5:00 pm (40 hrs/wk); other hours/schedule as needed

Job Summary The RN will provide disease-specific focused care management to patients with serious acute and/or chronic health events affecting bone health such as cancer treatment and dialysis, genetic bone disease and age-related osteoporosis, etc. RN responsibilities will include but are not limited to, skilled assessments, care plans and care plan implementation, patient education, and clinical evaluation or interventions.

This RN position focuses heavily on care management and coordination, including lab order entry, pre-appointment clinical preparation, obtaining and uploading external medical records, and will include patient follow up calls with emphasis on patient education for lab results and how these results impact the treatment plan.

EPIC inbox experience and task list proficiency highly preferred.
