Physical Requirements:

Regularly sitting, standing, walking, with some pushing, pulling, and/or lifting objects weighing up to 10 lbs and occasional lifting up to 50 lbs.

Shift:

Monday – Friday, Day Shift; 8:00 am – 5:00 pm (40 hrs/wk); other hours/schedule as needed.

Job Summary:

The RN will provide disease‑specific focused care management to patients with serious acute and/or chronic health events affecting bone health such as cancer treatment and dialysis, genetic bone disease and age‑related osteoporosis, etc. RN responsibilities include skilled assessments, care plans and their implementation, patient education, clinical evaluation or interventions, and coordination of care such as lab order entry, pre‑appointment clinical preparation, obtaining and uploading external medical records, and patient follow‑up calls with emphasis on education for lab results and their impact on the treatment plan.

Desired Proficiency:

EPIC Inbox Experience and Task List Proficiency (Highly Preferred).

Basic Computer Knowledge:

MS Windows, Word, Outlook.

Additional Notes:

No supervisory responsibilities.

Preferred Education:

ADN or BSN.
