Personal Service Coordinator/Case Manager III - Bilingual Preferred Turning Point of Central California, Inc. was founded in 1970 and has been transforming lives across California for over 50 years. This role supports clients in the community and office setting to accomplish daily living tasks, increasing independence and enabling them to continue living in the community.

Job Summary The Personal Service Coordinator/Case Manager III reports to the Personal Service Coordinator Supervisor and assists clients in achieving goals outlined in their Individual Service and Support Plan (ISSP).

Essential Job Responsibilities

Assist in the accomplishment of the case management plan by screening, evaluating, and implementing specific activities as identified in the ISSP.

Maintain case recording documentation per Rehab Medi‑Cal guidelines and other contract guidelines.

Attend weekly supervision meetings with the supervisor.

Provide direct case‑management services, including group/in‑individual rehab if needed.

Submit documentation and billing related to client services in a timely fashion.

Plan, coordinate, and facilitate services for Severely Mental Ill (SMI) clients.

Screen and evaluate potential clients for appropriate services.

Maintain professional relationships with clients, coworkers, governmental representatives, and others.

Communicate closely with agencies and significant others involved in rehabilitation.

Report unusual occurrences that may impact program operation or client/employer safety.

Complete required annual training hours.

Act quickly and efficiently in emergencies.

Drive company vehicles and transport consumers as needed.

Be physically present in the office.

Essential Job Requirements

Bachelor’s degree in social work, psychology, vocational rehabilitation, criminal justice, or related field from an accredited institution.

Minimum of one year experience managing rehabilitation for difficult-to‑serve families with barriers to independent community life.

Knowledge of best practices for clients with Severe Mental Illness (SMI) and dual‑diagnosis.

Effective written and spoken English communication.

Ability to work with diverse cultural, socioeconomic, and professional groups.

Ready for CPR and first aid training and certification.

Proficient in an Electronic Medical Records (EMR) system.

Intermediate computer skills in Office 365, Teams, Zoom, and other communication platforms.

Valid driver’s license and reliable, insured transportation in compliance with laws; agency insurer authorization required.

Pass a pre‑employment criminal background check.

Pass a pre‑employment drug screen and TB test.

Company Benefits

Pay Range:

Monolingual: Class 130 ($21.13 – $27.81/hour)

Bilingual: Class 135 ($22.21 – $29.23/hour)

Qualified non‑profit for the Public Service Loan Forgiveness (PSLF) Program.

Regular work schedule (Monday–Friday, 8 am – 5 pm).
