Position Title:
Senior LTSS Service Care Manager (RN).
Work Location:
Hybrid (2 days remote, 3 days member-facing, sourced from McAllen, Edinburg).
Assignment Duration:
6 months with possible extension.
Job Description Face-to-face assessments are completed three days a week, with two days devoted to documentation. Tasks include completing forms and assessments within turnaround times and conducting outreach activities.

Licensure:

Experience:
PDN experience, Pediatric experience, Home Health.
Must haves:
Pediatric experience.
