Home Health
provides compassionate, high-quality care to individuals in their homes, helping them maintain independence and improve quality of life. The program delivers skilled nursing and supportive services focused on safety, recovery, and chronic disease management while adhering to state and federal regulations and evidence-based best practices. Through a patient-centered approach, interdisciplinary collaboration, and a strong commitment to quality and compliance, Midland Care Connections Home Health supports patients and families with personalized care designed to meet their medical, functional, and emotional needs in the comfort of home.
Quality Facilitator
is responsible for overseeing and maintaining a comprehensive Quality Assessment and Performance Improvement (QAPI) program to ensure home health services meet agency, state, and federal regulatory and licensing standards. This role involves leading performance improvement initiatives, conducting audits and in-depth medical record reviews, analyzing quality and compliance data, and reporting findings to leadership with recommendations for corrective and preventive actions. The Quality Facilitator tracks publicly reported quality measures, adverse events, and regulatory reports, stays current with regulatory changes, and supports value-based purchasing requirements. They also provide education, training, and resources to staff, collaborate across departments, assist with billing audits and appeals, and actively promote a culture of quality, safety, and continuous improvement throughout the organization.
Qualifications Required
Registered Nurse (RN) degree with an active license in Kansas and Missouri
Knowledge and understanding of federal and state laws and regulations related to hospice, home health, home support, and palliative care programs
Strong communication skills with the ability to effectively engage staff at all levels of the organization
Demonstrated planning, analytical, and problem-solving skills
Ability to utilize technology to collect, analyze, and present quality and performance data
Valid Kansas or Missouri driver’s license with current automobile liability insurance
Reliable transportation and a motor vehicle record (MVR) that meets organizational insurability standards
Preferred
Previous experience in accreditation, quality improvement, education, and/or regulatory compliance
Prior involvement with performance improvement projects, audits, or regulatory surveys
Experience supporting quality initiatives in home health or related healthcare settings
