Primary Care Physician (PCP)

Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel.
Coaching for health: The PCP acts as a health coach, helping patients set short and long‑term health goals, partnering with them to work toward those goals, and frequently following up on them.
Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable for the patient and for their care team.
ESSENTIAL JOB DUTIES / RESPONSIBILITIES
Independently provides care for patients with acute and chronic illnesses encountered in older adults.
Full accountability for patient care and outcomes, seeking specialist consultation when needed while staying involved in the detailed care of the patient.
Engages with the hospitalist whenever one of their patients is in the hospital.
Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
Leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
For patients unable to come to the office—(hospital, SNF, LTC or homebound)—engages with the transitional care team and other resources as appropriate.
Active in center management, cover for other providers, and recruiting patients and additional providers for the company as needed.
Performs other duties as assigned and modified at manager’s discretion.
KNOWLEDGE, SKILLS & ABILITIES
Availability and Accessibility: Open and available for walk‑in visits and timely responses to phone calls and messages.
Service Orientation: Provides care that they would want for a family member or themselves to each patient at every interaction.
Evidence Based Medicine: Remains updated on evidence‑based medicine and recognizes the impact of lifestyle and nutrition.
Physician Leadership: Continuously improving leadership skills for patients, team, center, and company.
Quality: Willing to work with care teams toward achieving high quality outcomes and improving practice and center operations.
Influence: Competently and compassionately influences patients, teams, and self to achieve best outcomes.
Self‑Care: Takes care of themselves physically, mentally, socially, and spiritually to better care for patients.
Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
Willingness to travel locally and occasionally regionally/nationally for training or recruiting.
Fluency in English (verbal & written); other languages may be required in some centers.
Requires independent judgment.
EDUCATION AND EXPERIENCE CRITERIA
MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
State medical license in the applicable state(s); current active license required.
Board certification preferred; board eligibility required. Maintenance of certification through MOC/CME required.
Current DEA number for Schedule II‑V controlled substances.
Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days.
PAY RANGE $214,700 - $306,714 Salary
