Job Summary Provides specialized wound care services in home settings, nursing homes, and assisted living facilities. Responsibilities include assessments, care plan development, patient and caregiver education, and collaboration with healthcare teams to promote wound healing.
Key Responsibilities
Conduct wound assessments and develop care plans.
Perform wound treatments and document care.
Supervise nurses and medical assistants.
Collaborate with healthcare teams and educate patients and caregivers.
Ensure compliance with policies and safety protocols.
Qualifications Licensed MD/DO with at least 2 years of relevant experience. Requires a Master’s or Doctoral degree in Nursing as a Nurse Practitioner (if applicable). Certification in Wound Care must be obtained within 6 months of hire.
Physical & Mental Requirements Involves physical effort, including lifting and mobility tasks, and mental acuity for assessments and quick decision‑making. Must adapt to various environments and manage stress effectively.
Work Conditions & Travel Field‑based role with travel to patient locations, exposure to biohazards, and requirement of reliable transportation. Travel expected up to 25%.
