The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.
Provides specialized wound care in home, nursing home, and assisted living settings for chronic and complex wounds.
Conducts assessments, develops care plans, educates patients/caregivers, and collaborates with healthcare teams to promote healing.
Requires a Master’s or Doctoral degree in Nursing, valid NP licensure, wound care certification within 6 months, and minimum 2 years of relevant experience.
Field-based role involving travel, physical activity, and exposure to biohazards, with necessary physical and mental stamina.
Performs procedures, maintains documentation, ensures compliance with safety and HIPAA policies, and coordinates care with providers.
Strong assessment, communication, and adaptability skills are essential.
