Position Description
Provides individual, family, and group counseling services to patients to address various mental health and wellness-related issues. Conducts assessments and helps patients to problem‑solve areas of need. Helps to develop coping strategies and action‑based goals and objectives for patients. May specialize in a particular area of counseling.
Major / Essential Functions
Provide brief, evidence‑based therapy to patients referred through collaborative care pathways.
Supervise mental health fellows weekly; provide feedback and assess progress using structured tools.
Coordinate with PCPs, case managers, and other clinical staff to ensure continuity of care.
Participate in team meetings, program development, and clinical documentation audits.
Assist in the training and onboarding of new fellows, including policy and documentation orientation.
Promote trauma‑informed, culturally competent care across service sites.
Required Qualifications
Education: Master's degree in psychology, counseling, social work, or related field.
3 years of related experience.
Licenses / Certificates: LPC, LPC‑A, LCSW, LMSW (must be working towards LCSW), LMFT, LMFT‑A, LPA.
Preferred Qualifications
Experience with CoCM or integrated care strongly preferred.
