Position Summary We are seeking a compassionate and dedicated
Licensed Practical Nurse (LPN)
to join our healthcare team in a
PRN
(as-needed) capacity. As an LPN you will provide quality nursing care to patients under the supervision of registered nurses (RNs) and physicians ensuring the highest standard of patient care in a collaborative and supportive environment.
Key Responsibilities
Provide direct patient care including taking vital signs, administering medications, performing treatments and assisting with daily activities.
Monitor and assess patients' conditions, documenting changes and reporting concerns to the supervising RN or physician.
Collaborate with healthcare team members to develop and implement patient care plans.
Maintain accurate up-to-date patient records and documentation in compliance with facility policies and regulations.
Ensure a clean, safe and comfortable environment for patients at all times.
Provide emotional and psychological support to patients and their families.
Adhere to all facility policies, procedures and standards of practice.
Assist in patient education and counseling regarding health and wellness.
Maintain patient confidentiality and uphold HIPAA standards.
Required Qualifications
Current and valid
LPN license
in the state of
Connecticut .
CPR/BLS certification (required).
Minimum of 1 year of nursing experience.
Strong clinical communication and organizational skills.
Ability to work independently and as part of a team.
