This role offers the opportunity to deliver one‑on‑one care, develop strong patient relationships, and grow within a supportive, family‑like culture.
Hours: Full‑time M‑F.
Responsibilities
Abide by 3HC's Compliance Program and Code of Ethics.
Create positive experiences for patients, staff, and visitors, demonstrating courtesy, tact, and knowledge of agency services.
Manage assigned cases and assist the office in achieving positive patient outcomes, strategic plans, and budget goals.
Conduct complete patient assessments to identify appropriate care needs.
Document care electronically and download daily records.
Complete OASIS at required points such as admission, resumption, and recertification.
Provide regular supervision to Licensed Practical Nurses and paraprofessionals according to regulatory and CMS/Hospice guidelines.
Communicate patient issues timely through appropriate chain of command.
Ensure correct billing of services for 3HC.
Demonstrate cost‑effective use of agency resources, monitoring waste and ensuring safe supply usage.
Qualifications
Registered Nurse licensed in North Carolina.
Minimum one year of professional nursing experience in an acute care setting preferred.
Preferable one year of Home Health experience.
Valid North Carolina Driver's License.
