Role Summary The Home Health Nurse, RN is responsible for administering skilled nursing care to patients requiring intermittent professional nursing service.
Registered Nurse (RN) Essential Job Functions
Abides by and supports 3HC's Compliance Program and Code of Ethics.
Creates positive experiences for internal and external customers, displaying courtesy and knowledge of services.
Manages assigned cases and assists office with achieving positive patient outcomes and strategic planning.
Conducts complete assessments of patients to identify appropriate care needs.
Documents in the medical record electronically and downloads daily.
Completes OASIS at specific points (admission, resumption of care, recertification, etc.) and supervises Licensed Practical Nurses and paraprofessionals.
Maintains regular communication with supervisors and reports patient problems timely.
Ensures 3HC can bill for services appropriately.
Demonstrates cost‑effective use of agency resources, monitoring waste, and safe use of supplies and equipment.
Qualifications
Registered Nurse licensed to practice in North Carolina.
Minimum of one year experience as a professional nurse in an acute care setting preferred.
Prefer a minimum of one year Home Health experience.
Valid North Carolina Driver's License.
