Job Summary
Serves as liaison to coordinate and link hospital resources, acts as interface with third‑party payors/home health, coordinate research projects, and provide adequate follow‑up and rehabilitation for patient; also serves as clinical liaison to identify compliance/quality improvement problems. Provides professional nursing services according to established standards and practices. Key Job Responsibilities
Attend rounds discussing patient plan with medical staff; maintain treatment protocols and monitor for compliance and results. Facilitate the timely and appropriate completion of medical records and forms; arrange for monthly inpatient chart review. Maintain follow‑up and coordinate with patient/family/hospital departments regarding all aspects of patient care needs (e.g., patient transfer, discharge planning, rehab services, outpatient visits). Provide professional nursing care in accordance with established hospital/department policies and procedures; demonstrate knowledge and skills to provide care and meet age‑specific patient needs. Interact and communicate in a customer‑responsive and professional manner; work collaboratively and coordinate activities with hospital/medical staff and community agencies to integrate and implement activities. Participate in monitoring/reviewing quality improvement and utilization review activities; serve as contact person regarding all inquiries and reports. Prepare and maintain required statistics, reports, records and files. Coordinate research projects. Participate on various nursing and hospital committees. Physical Demands

Graduate of an approved nursing school/program (required). Bachelor’s Degree or BSN (required). Preferred: Qualified by education, training or experience to work with the neonate, infant, toddler, pre‑schooler, school‑age, adolescent, adult and/or geriatric patient population as specialty assignment dictates. Work Experience
Minimum 3 years of clinical experience in an acute care hospital (required). Experience and skills applicable in area of assignment (required).
