Guaranteed Hours: 40 hours per week
Experience: Minimum of 2 years
License: Must possess a valid RN license.
Certifications: BLS
Must-Have: Prior experience with Dignity Health
Responsibilities
Perform case management and care coordination in all areas of the hospital.
Handle both utilization review and discharge planning responsibilities.
Engage with various patient demographics, including those dealing with all payers, the uninsured, and the homeless.
Conduct initial assessments, IQ (Interqual), and discharge (DC) planning.
Utilize Cerner for charting.
Collaborate with hospitalists or intensivists who are in-house.
Adapt to weekend staffing requirements; at least two weekends per month.
Adhere to dress code requiring business casual attire.
Participate in 5 days of care coordination new employee orientation.
Ensure effective communication and teamwork within the hospital setting.
Reprioritize tasks throughout the day as needed.
Training provided in Interqual; both DC Planning and Utilization Management are essential.
Note: Holidays are not required, but willingness to work is appreciated if necessary. The hospital may cancel up to 4 shifts per 13-week assignment.
