Our team is seeking a Licensed Practical Nurse to provide comprehensive, client-centered care within a residential setting. This essential role supports our mission of delivering high-quality mental health services and medical oversight.
Responsibilities
Complete drug and past medical history.
Monitor vital signs and complete nursing assessment upon admission and as needed.
Schedule and assist medical director/nurse practitioner with new client physicals.
Obtain all client medication and update client medication sheet as needed.
Screen clients for co-occurring disorder and refer to the psychiatric clinic.
Administer client medication and observe client health status.
Provide medical case management for client visits to outside resources.
Triage client medical complaints.
Prepare and maintain clinical and program records.
Participate in clinical staffing and team meetings.
Schedule client clinic appointments.
Promote and encourage good communication with counseling staff to ensure optimal holistic care of clients.
Arrange client HIV education, testing and counseling.
Conduct educational groups on health-related topics including co-occurring disorders.
Communicate with other medical personnel in case of emergencies.
Maintain facility medications.
Transcribe physician's orders.
Submit reports as required.

Maintain client rights as defined by DCF.
Maintain clinical records in accordance with Agency and other regulatory agencies standards.
Follow all established Agency policies and procedures.
Maintain the highest standards in ethical conduct and confidentiality.
Model ideals of integrity, loyalty, and teamwork.
Sustain a positive, caring, supportive, recovery-oriented environment.
Knowledge, Skills & Abilities
Read, analyze, and interpret professional journals, technical procedures or governmental regulations.
Write reports and present information effectively.
Respond to questions from clients and staff.
Calculate proportions, percentages, and volumes as required.
Solve problems and adapt to varied situations with limited standardization.
Interpret written, oral, diagrammatic, or scheduled instructions.
Perform CPR and other First Aid procedures.
