People Incorporated is seeking a
Case Manager II
to join our
Waiver Case Management Anoka County
team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self‑awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long‑term relationships with their clients and operate autonomously, creating their own schedules. Waiver Case Management involves more behind‑the‑scenes work supporting client needs than time meeting with clients.
We value work‑life balance and have manageable caseloads that are around 30 clients on average!
Hours:
Full‑Time, Monday‑Friday, business hours 8:00‑4:30 pm
Locations:
Anoka County | Minnesota | Hybrid
Hiring Range:
$24.25 – $25.75/Hourly based on qualifications
Hiring Bonus:
$1,000 to external candidates

Responsibilities
Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
Assess client needs and ensure continuity of care.
Make ongoing referrals to resources based on assessment and current client need.
Maintain necessary daily records for continuity of service and reports to funding agencies.
Document all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
Attend client monthly meetings to monitor services.
Requirements
Valid driver’s license, excellent driving record and current auto insurance.
Education and Experience Required
Bachelor’s Degree from an accredited 4‑year college with a major in social work, psychology, sociology or a closely related field; OR
Bachelor’s Degree from an accredited four‑year college with a major in any field AND One year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
Preferred Education and Experience
Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND One year of experience in the education or treatment of persons with developmental disabilities or related conditions.
