LPN Resident Wellness Coordinator

LPN Residents Wellness Coordinator - Assisted Living Located in Staten Island, NY

Salary Rate $30/hr *based on experience*

LPN Resident Wellness Coordinator Responsibilities:

Monitor and assess residents’ health and report changes in condition

Administer and oversee medications and treatments

Develop, update, and manage individualized care/service plans

Coordinate medical services, appointments, and outside providers

Communicate with residents, families, and healthcare professionals

Supervise and train caregivers and medication staff

Ensure accurate documentation and regulatory compliance

Monitor infection control, safety, and incident reporting

Support admissions, transitions of care, and discharges

Advocate for resident wellness, safety, and quality of life

LPN Resident Wellness Coordinator Qualifications:

Must have New York LPN license

Assisted Living / Healthcare experience

Strong oral and written communication skills

Strong computer skills

Excellent organizational skills
