Wellness Coordinator (Project Management)
Join Our Team as a Wellness Coordinator (LPN)! Are you a compassionate, organized, and clinically skilled LPN who enjoys building relationships, supporting residents' wellness, and partnering with a care team to deliver exceptional service? Stoney River Marshfield is seeking a Wellness Coordinator (LPN) to support resident care, clinical coordination, and overall wellness programming within our community. What You Do (Your Wellness Superpowers)
Coordinate Resident Wellness & Care – Support resident assessments, care planning, and ongoing wellness monitoring.
Coordinate clinical services to ensure resident needs are met in a timely and compassionate manner.
Administer medications and treatments in accordance with physician orders and state regulations.
Monitor changes in resident condition and communicate updates to families and providers.
Support Compliance & Quality Care
Ensure clinical documentation is accurate, complete, and compliant with state and company standards.
Assist with audits, surveys, and quality assurance initiatives.
Support infection control practices and medication management protocols.
Help maintain survey readiness at all times.
Partner with the Care Team
Collaborate with caregivers, leadership, and interdisciplinary team members.
Provide guidance, education, and support to caregiving staff as needed.
Serve as a clinical resource to promote best practices and resident‑centered care.
Foster a positive, team‑oriented environment.
Communicate & Build Relationships
Serve as a point of contact for resident wellness questions and concerns.
Communicate effectively with residents, families, providers, and internal teams.
Support admissions, move‑ins, and transitions of care as applicable.
What You Bring to the Table (Beyond Your Clinical Expertise) Experience & Credentials
Current LPN license in good standing (required).
Long‑term care, assisted living, or senior living experience preferred.
Experience with medication administration and resident assessments.
Skills & Strengths
Strong organization, time‑management, and follow‑through skills.
Compassionate, resident‑focused approach to care.
Clear and professional written and verbal communication.
Ability to multitask and adapt in a fast‑paced environment.
Proficiency with electronic medical records (PointClickCare experience preferred).
Physical Requirements
Ability to lift up to 30 lbs and assist with resident care as needed.
Ability to walk, stand, bend, and lift throughout the shift.
Flexible availability, including weekends, holidays, and on‑call as needed.
