An opportunity is available for a Clinical Pharmacist to join a hybrid clinical/staff team in State College, PA. This role combines approximately 60% clinical duties with 40% staffing responsibilities, providing a dynamic and varied work experience. The clinical focus centers on Population Health and Ambulatory Care, particularly managing medication regimens for patients with chronic conditions such as diabetes. This position offers the potential to transition into a fully clinical role as the program continues to grow. Key Responsibilities: Deliver clinical pharmacy services with emphasis on patient medication management in outpatient and ambulatory care settings Collaborate with healthcare teams to optimize medication use and improve health outcomes in target disease states Provide staffing support including medication dispensing and patient counseling Assist in developing and expanding clinical pharmacy programs within the health system Qualifications and Desired Experience: Licensed Pharmacist in Pennsylvania Experience in ambulatory care or inpatient pharmacy practice preferred Completion of a pharmacy residency strongly preferred Proficiency with Meditech or similar clinical IT systems is a plus Strong commitment to patient care and clinical program growth Schedule: Variable day and afternoon shifts Primarily 8-hour shifts This permanent position offers a unique chance to work in a smaller hospital setting where clinical services are expanding.
