A dynamic opportunity is available for a Clinical Pharmacist in a hybrid role combining clinical and staffing responsibilities. This position involves approximately 60% clinical work focused on Population Health and Ambulatory Care, particularly managing medication regimens for targeted disease states such as diabetes, and 40% staffing duties. The program is designed to grow, with potential to transition into a fully clinical role as the clinical services expand. Responsibilities: Provide clinical pharmacy services primarily in Population Health/Ambulatory Care settings Manage and optimize medication regimens for patients within the health system Collaborate with healthcare teams to improve patient outcomes for chronic conditions such as diabetes Support staffing needs of the pharmacy department as required Contribute to the development and expansion of the clinical pharmacy program Qualifications: Licensed Pennsylvania pharmacist Experience in ambulatory care or inpatient pharmacy preferred Completion of a residency program is highly desirable Strong clinical knowledge and patient care skills Adaptability to work both clinical and staffing shifts Location: State College, Pennsylvania Schedule/Hours: Variable days/afternoons, primarily day shifts 8-hour shifts Information Technology: Experience with Meditech preferred This direct-hire position offers the chance to join a committed department focused on scaling clinical pharmacy services within a smaller hospital environment.
