A healthcare facility in Columbia, SC is currently seeking a Clinical Pharmacist with experience in clinical pharmacy settings to provide expert pharmaceutical care. This role focuses on supporting a rehabilitation setting and offers a day shift schedule with typical weekday hours. Key Responsibilities: Dispense medications accurately and efficiently in accordance with state and federal regulations. Review patient medication orders for appropriateness, interactions, and contraindications. Collaborate with healthcare teams to develop and implement individualized medication plans. Provide clinical interventions and patient education to optimize therapeutic outcomes. Monitor patient responses to medications and adjust therapies as needed. Qualifications: Licensed pharmacist with strong clinical pharmacy skills. Experience working in rehabilitation or similar healthcare settings preferred. Ability to assess complex medication regimens and provide evidence-based recommendations. Excellent communication skills to effectively interact with patients and healthcare professionals. Detail-oriented and capable of maintaining accurate records.
