Key Responsibilities
Conduct client assessments to determine needs and create tailored case management plans
Collaborate with healthcare providers, social services, and community organizations to coordinate care
Monitor client progress and adjust care plans as needed to achieve goals
Advocate for clients, ensuring they have access to necessary services like healthcare, housing, and financial assistance
Maintain confidential, accurate client records and documentation
Facilitate communication between clients, their families, and service providers
Provide crisis intervention and immediate support during critical situations
Qualifications
Bachelor’s degree in Social Work, Psychology, or a related field (MSW preferred)
2+ years of experience in case management or social services
Strong knowledge of social services, healthcare, and community resources
Excellent organizational, communication, and problem-solving skills
Ability to handle a varied caseload independently
Strong interpersonal skills and experience working with diverse populations
Valid state licensure or certification as required
Competitive hourly pay: $18 - $24
