ChenMed is a physician‑led, mission‑driven primary care organization that delivers high‑quality, affordable VIP care for seniors. We are expanding rapidly and need dedicated Primary Care Physicians to join our team. Responsibilities
Accountable for patient outcomes and cost‑effectiveness for a panel of up to 450 patients. Act as a health coach, helping patients set and achieve short‑ and long‑term health goals. Simplify and prioritize care to make behavior change actionable for patients and the care team. Function independently and collaboratively with other health professionals, including hospitalists, case managers, and the transitional care team. Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document all patient interactions. Lead the care team (care promoter, facilitator, coordinator) for patients who visit the office. Engage with hospitalists for patients admitted to any hospital, regardless of affiliation. Support center operations, cover for other providers when needed, and help recruit patients and additional providers. Perform other duties as assigned by management. Knowledge, Skills & Abilities
Availability and accessibility to build patient trust through walk‑in visits and timely communication. Service orientation, delivering care they would wish for a family member. Up‑to‑date knowledge of evidence‑based medicine, lifestyle, nutrition, and preventive interventions. Strong physician leadership and continuous improvement of clinical practice and center operations. Influence patients, teams, and self to achieve the best outcomes. Commitment to self‑care and physician wellness. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and related software. Willingness to travel locally, regionally, or nationally for training, recruiting, or other duties. Fluency in English (verbal and written); other languages may be required for specific locations. Independent judgment and adherence to regulations. Education and Experience Criteria
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Current, active State Medical License for the state(s) in which the physician will practice. Board certification preferred; Board eligibility required, with ongoing maintenance of certification. Active DEA number for schedule II–V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within 90 days of employment. Pay Range
