Family Specialist (ACT/FACT Team Member)

FlexStaff is seeking a Family Specialist for our client, to join their multidisciplinary team dedicated to empowering individuals with severe and persistent mental illness.
This position will provide direct, family-focused services within an Assertive Community Treatment (ACT/FACT) framework. This role is pivotal in supporting our clients' recovery and well-being in their communities.
RESPONSIBILITIES
Provide direct services with a strong focus on family engagement and support for individuals with severe and persistent mental illness.
Manage an assigned caseload, including timely follow-up on new referrals, admissions, and discharges, ensuring comprehensive care coordination.
Collaborate in developing, reviewing, and updating individualized treatment plans, comprehensive assessments, and crisis/wellness plans (e.g., every 6 months or annually/after crisis).
Maintain accurate and timely case records, including progress notes (within 48 hours), CAIRS entries, and various assessments (e.g., HML, Substance Abuse, Suicide/Violence Risk). Ensure all documentation meets regulatory standards.
Provide weekly progress updates and complete service verifications for Assisted Outpatient Treatment (AOT) clients, reporting significant events promptly.
Conduct all assigned client visits (within 7 work hours), including providing transportation and coordinating hospital discharge paperwork.
Conduct therapeutic group sessions aligned with professional discipline.
Process service dollar, WRAP fund requests, and travel expenses for client support.
Work effectively within the ACT/FACT team, utilizing agency IT applications, and adhering to all compliance and safety policies.
QUALIFICATIONS
Master’s degree or higher in Social Work, Psychology, or Rehabilitation Counseling (C.R.C. preferred for Rehabilitation Counselors).
Minimum of 1 year of experience in a treatment or rehabilitation setting.
Valid and clean NY State Driver's License.
Successful completion of fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
Excellent interpersonal and communication skills (written and verbal).
Strong organizational and time management abilities.
Proficient documentation skills and attention to detail.
Knowledge of treatment and rehabilitation methods for severe and persistent mental illness, including recovery principles and psychiatric rehabilitation.
Skilled in individual and group counseling techniques.
Proficiency with office software and agency IT applications.
Ability to work independently and collaboratively within a team.
Professional demeanor and commitment to maintaining credentials.
