Contract Speech Therapist, Home Health for a W2 Contract position based in Santa Clara, CA.
Schedule:
Full time, Monday through Friday, 8:30am-5:00pm, with occasional weekends required
Assignment Duration:
13 Weeks with high likelihood of extending
Compensation:
Daily Rate:
This position offers an hourly rate between $25.00 to $28.00 per hour which equates to $1,000 to $1,120 per week (based on a 40-hour workweek).
Per Diems:
Lodging: $192 per day, Meals & Incidentals: $92 per day and Auto Stipend: $25 per day. Per diems and Auto Stipend are paid daily, 7 days a week, totaling $2,163 per week.
Total Weekly Compensation (Daily Rate based on a 40-hour work week + Per Diems) : Between $3,163 to $3,283 per week

Summary The Home Health Speech Therapist provides comprehensive speech-language pathology services to patients in their homes, supporting assessment, treatment, and education for individuals with speech, language, cognitive, and swallowing disorders. This role serves a defined coverage area including north Santa Clara County and San Mateo County, with care delivery extending to Gilroy and coastal communities as assigned. The Speech Therapist works independently while collaborating with an interdisciplinary care team to deliver high-quality, patient-centered care in compliance with clinical standards and regulatory requirements.
Essential Functions
Conducts in-home evaluations and develops individualized treatment plans for patients with speech, language, cognitive, and swallowing disorders
Provides therapeutic interventions and patient/family education in accordance with physician orders and plan of care
Documents evaluations, treatments, progress, and outcomes accurately using electronic charting systems
Utilizes shared clinical equipment in accordance with agency protocols and infection control standards
Collaborates with nurses, therapists, physicians, and care coordinators to ensure continuity of care
Monitors patient progress and adjusts treatment plans as clinically indicated
Educates patients and caregivers on exercises, strategies, and safety recommendations
Maintains compliance with federal, state, and organizational regulations and policies
Manages scheduling and travel within the assigned coverage area to meet patient care needs
Participates in case conferences, quality improvement activities, and required training
Qualifications
Active Speech-Language Pathologist (SLP) license required
Current Basic Life Support (BLS) certification required
Experience in a home health or community-based setting preferred
Experience using electronic charting systems required
Experience working with shared clinical equipment requiredStrong organizational, communication, and time-management skills
Ability to work independently across a geographically diverse coverage area
