What You Will Do:

Coordinate the processes of assessment, planning, implementation, and evaluation of patient care in collaboration with patients, families, physicians, and support services.

* Conduct comprehensive and ongoing physical and psychosocial assessments, including functional and nutritional screenings.

* Use evidence-based assessment tools and analytical models to identify patterns and determine nursing diagnoses.

* Collaborate with interdisciplinary teams to develop individualized plans of care with clearly defined interventions and outcomes.

* Implement care interventions, including treatments, medications, and patient/family education, ensuring safe and timely delivery.

* Delegate and coordinate patient care with support services while maintaining oversight and accountability.

* Evaluate effectiveness of interventions and revise care plans based on reassessments.

* Actively involve patients and families in care decisions and discharge planning.

* Maintain thorough documentation and adhere to nursing practice standards, safety regulations, and compliance requirements.
