Description:

- We are seeking a compassionate and skilled Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team.

- This role involves providing holistic, person-centered nursing care to individuals with serious mental illness (SMI) and co-occurring disorders in a community-based setting.

ESSENTIAL DUTIES & RESPONSIBILITIES:

- Provide medication education, administration, and monitoring to ACT clients.

- Conduct comprehensive nursing assessments, focusing on both physical and mental health needs.

- Work closely with psychiatrists, therapists, case managers, and peer specialists to develop and implement individualized treatment plans.

- Support clients in managing chronic health conditions and accessing medical care.

- Offer health education on topics such as medication adherence, nutrition, diabetes management, and harm reduction.

- Participate in daily ACT team meetings and interdisciplinary care planning.

- Respond to crises and provide on-call support as needed.

- Ensure compliance with state and federal regulations, maintaining accurate clinical documentation.

REQUIRED EDUCATION AND EXPERIENCE:

- Licensed Registered RN within the state of New York.

- Must have excellent verbal, written and interpersonal communication skills.

PREFERRED QUALIFICATIONS & SKILLS:

- Experience working with behavioral health and substance abuse clients ideal

- Experience working in ACT, mobile crisis, or forensic mental health settings
