Job Title: Administrative Assistant - Case Management Team
Location: 8700 Beverly Blvd., Becker Bldg. Suite 220, Los Angeles, CA 90048
Job Duration: 13 weeks
Shift: Day - 5x8-Hour (08:00 AM - 05:00 PM)
Job Summary:
* We are seeking a detail-oriented, computer-savvy Administrative Support Assistant to join our outpatient case management team. This role is highly administrative and supports Case Managers in coordinating patient care and ensuring seamless referral processes. The ideal candidate is a team player with excellent communication skills and experience working in a medical office setting.
Key Responsibilities:
* Coordinate follow-up care and appointments with PCPs, specialists, pharmacists, etc.
* Process referrals for home health, SNF, and DME as requested by Case Managers
* Track patient admissions and initiate related referral processes
* Support case management with coverage confirmation and clinical request submissions to health plans/medical groups
* Facilitate care coordination for high-risk/high-cost patients, including referrals to community and ancillary services
* Create and submit HMO referrals/authorizations under Case Manager guidance
* Accurately document all patient information in designated systems
* Verify patient health plan benefits
* Send faxes and manage various administrative duties to support the case management team
* Assist in maintaining accurate monthly reports and compiling patient letters
* Distribute daily staffing schedules and provide administrative support for meetings and presentations
* Perform other duties as assigned
