Role Description
The Regional Quality Programs RN (QPRN) is responsible for monitoring areas pertaining to the health and welfare of individuals receiving Home and Community Based Services (HCBS) programs. This position works closely with individuals receiving HCBS services, division contracted provider agencies, and regional office team members.
What You’ll Do
Maintain knowledge of current nursing practices related to individuals with developmental disabilities.
Familiarity with Division’s HCBS 1915(c) Waiver service requirements, Individual Support Plan (ISP) requirements, and Person-Centered Planning guidelines.
Proficiency in database analysis for trend reporting related to care planning, delivery, and consumer health and safety.
Participate in Transition Planning Meetings virtually, in-person, or phone, for consumers entering community placement to provide clinical consultation review and feedback to transition team members as needed/requested.
Conduct Clinical QA/ Review and analysis of reportable events in areas such as choking, falls, medication errors, ER visits, and hospitalizations.
Maintain working knowledge of the IntellectAbility, Health Risk Screening (HRS) system.
Function in the role of HRS Clinical Reviewer.
Participate in Regional Office Mortality Review Committee meetings.
Coordination with internal processes to ensure a multidisciplinary team approach for consumer health and safety.
Timely and accurate submission of work products in accordance with regulations and guidelines.
Manage a caseload of 250+ consumers and meet all workload requirements within a 40-hour work week.
Participation in QPRN Meetings and other assigned duties.
Essential Functions
Strong organizational, decision-making, and communication skills.
Proficiency in computer skills, including Microsoft products.
Utilize and navigate division data systems including but not limited to: CIMOR, EMT 2 system, Tableau, Integrated Quality Management Functions (IQMFD), DMH ADHOC Reporting (Reports Manager).
Ability to build working relationships with a variety of providers and/or other stakeholders.
Knowledge of DD/DMH vision, guidelines, Department Operating Regulations, policy, procedure, and Code of State Regulations.
Willingness to travel to various locations throughout the work week.
Adherence to facility policies for attendance and coverage.
Typical Qualifications
RN Licensure in good standing to practice in the State of Missouri.
Valid Driver’s License.
