Community Care Physicians . This is a full‑time, outpatient position located at our state‑of‑the‑art building Wellness Way in Albany, NY. The position is eligible for a $5,000 recruitment/retention bonus.
What You Will Do As a Physical Therapist, you will provide direct patient care and integrate elements of patient/client management, examination, evaluation, diagnosis, prognosis, and interventions to maximize functional outcomes. You will also document treatments and progress according to professional, department policies and procedures.
Responsibilities
Formulate comprehensive treatment programs by evaluating patient history, impairments, disability, and functional status.
Prioritize patient care needs.
Accurately assess patient needs across the continuum of care and provide timely disposition, equipment, education, and treatment recommendations.
Complete all documentation in accordance with professional, regulatory, and facility standards.
Maintain concise, legible, and compliant clinical and payer documentation.
What You Will Need
Graduate of an approved Physical Therapy program with Bachelor, Master’s, or Clinical Doctorate degree.
Current NYS Physical Therapist license and registration.
1 year physical therapy experience preferred (new graduates welcome).
Effective verbal and written communication skills.

Compensation $78,000.00 – $118,577.28 annually (base salary). CCP’s compensation packages include competitive benefits and opportunities for overtime.
