Job Overview The Wellness Director reports to the Executive Director and is responsible for the administration of nursing services. He/she directs, plans, and coordinates the services and activities of professional nursing and auxiliary nursing personnel in rendering resident care. The Wellness Director will interpret policy and regulations to all nursing personnel and ensure compliance. Responsibilities include:

Ensuring clinical services are provided in accordance with state regulations, accepted clinical standards, and Community policies & procedures.

Assisting in the coordination and management of nursing and personal care associate services provided to residents.

Providing nursing services to residents, including resident instruction, wellness counseling, health promotion, disease prevention.

Providing care and services to residents with chronic and stable conditions.

Verifying resident medication regimen with physicians or healthcare practitioners for residents who require supervision.

Assisting residents with activities of daily living (bathing, dressing, eating, toileting, hygiene, mobility) as needed.

Maintaining awareness of federal and state laws and regulations regarding assisted living/memory care and the state’s nursing commission; ensuring regulatory compliance.

Coordinating care through the move‑in process, pre‑move‑in wellness review, initial Resident Service Plan Development, and ongoing Wellness Plan Reviews.

Managing coordination with residents, families, and outside healthcare agencies including referrals.

Conducting staff training, education, supervision, orientation, and continuing training per community policies.

Reviewing documentation and communication of Wellness Associates; providing accurate documentation as required.

Delegating RN tasks within state regulations and community policy.

Being available after hours for resident needs and off‑site assessments as required.

Maintaining Infection Control compliance on immunizations and infectious disease management.

Assisting move‑out/discontinuation of services for residents no longer requiring services.

Completing routine auditing of all resident care systems in accordance with QA tools.

Preparing monthly Wellness staffing schedules and weekly reports to the General Manager, including summaries for governing authorities.

Participating in other resident care services and community duties as assigned by the Executive Director.

Following all emergency procedures and safety policies.

Communicating effectively with the Executive Director and community team members.

Qualifications

Registered Nurse or Licensed Practical/Vocational Nurse licensed in the state.

A baccalaureate degree in nursing with a minimum of two (2) years of full‑time clinical experience, at least one (1) year in home healthcare or community health programs involving care of the sick at home; or

A diploma or associate’s degree in nursing with at least four (4) years of full‑time clinical experience within the past ten (10) years, with at least one (1) year in home healthcare or community health programs involving care of the sick at home.

Experience and passion for working with seniors.

Physical capacity to perform tasks such as pushing, bending, lifting up to 60 pounds.

Ability to maintain positive relationships with residents, families, peers, and staff.

Good physical and emotional health, free of communicable diseases.

Ability to perform frequent lifting, pushing, pulling, moving equipment, bending, and stooping.

Ability to spend long periods on foot.

Quick reaction in any situation.
