The main job duties include responding to and carrying out pre‑admission screenings for patients referred to the rehab program that result in a timely decision as to whether or not the patient is an appropriate candidate for the inpatient rehab program.
Secondarily the job duties include developing, implementing, and adhering to best practice patient attraction strategies as a means to promote the features and benefits of the rehab program to patients, families, and the community. The amount of time spent performing on the secondary task will vary based on volume of activity of referrals the program receives.
Position Summary
The Admissions Liaison is responsible for conducting a comprehensive assessment of patients referred for inpatient acute rehabilitation services, coordinating the gathering of patient information to present in a timely manner to the Medical Director for review, and communicating quickly and effectively with all referral sources at every stage of the referral process. Minimum Requirements Experience: 2 years of clinical experience, to include direct patient in a rehabilitation setting Education: Bachelor's degree, or greater, as required by licensure Certifications :Licensed PT, RN, OT or SLP.
