Seeking a qualified Nurse Practitioner to provide home-based patient care in the Beaufort area, including Hilton Head Island.
As a Provider, you will evaluate high-risk patients, develop care plans, coordinate with interdisciplinary teams, and deliver education to improve outcomes.
The full‑time, salary‑based role offers a monthly bonus, with shifts including 12‑hour (7 days on/off) and 8‑hour (Monday‑Friday) options.
