Yavapai County Community Health Services is seeking a Counselor to join our team. Working under minimal supervision, the Counselor will be an integral part of the healthcare team, responsible for delivering integrated, short-term, patient-focused assessment, intake, and counseling services to patients at the Community Health Center of Yavapai (CHCY) in an outpatient setting. We have openings in our Prescott, Prescott Valley, Cottonwood and Ash Fork locations.
The ideal candidate will possess training and experience working across the lifespan, including pediatric patients. Bilingual in English/Spanish is preferred.

Major Duties, Responsibilities
Provides comprehensive patient focused services within an integrated primary medical care model and documents in accordance with regulations for licensure in Arizona, State and Federal Program Guidance Manuals and the requirements of other payors.
Performs various duties to include assessment, diagnosis, planning of intervention, treatment and evaluation of therapy.
With input from the patient and other staff, makes an initial assessment, develops the treatment plan that addresses clinical goals and objectives (targeted behavior change) and establishes realistic time frames to achieve those goals. Coordinates and consults with primary care providers and other health care providers as needed.
Provides intake and counseling services to patients in individual or group settings based on treatment plan goals.
Documents services provided to patients in accordance with department standards including required consents forms, progress notes, treatment plans and discharge plans.
Performs other job-related duties as assigned.
Minimum Qualifications Education:
Master’s degree in Behavioral Science including, but not limited to, Marriage and Family Therapy, Psychology, Counseling, Social Work.
Experience:
A minimum of three (3) years of relevant experience in social work or counseling.
Additional Requirements:
Must possess a valid Arizona driver’s license. Must possess Certification as a Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW). Must possess appropriate licensure through the Arizona Board of Behavioral Health Examiners to perform required duties. Must possess a current fingerprint clearance card issued by the Arizona Department of Public Safety.
