LTSS Family Care‑Service Coordinator Elevance Health is seeking a dedicated LTSS Family Care‑Service Coordinator to serve as a case manager for clients in the Family Care program. This field‑based role requires travel to client sites, occasional office attendance, and a commitment to providing person‑centered support for older adults and adults with disabilities.

Southwestern Wisconsin counties: Pepin, Buffalo, Trempealeau, Jackson, Clark, La Crosse, Monroe, Juneau, Vernon, Sauk, Richland, Crawford, Grant, Iowa, Lafayette, Green.

Responsibilities

Conduct face‑to‑face program assessments using various tools with pre‑defined questions and apply motivational interviewing techniques.

Identify members with potential clinical health care needs and coordinate care as the single point of contact with the clinical healthcare management and interdisciplinary team.

Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, and/or disabilities to ensure cost‑effective and efficient utilization of long‑term services and supports.

Document short and long‑term service and support goals in collaboration with the member’s care team, including caregivers, family, natural supports, service providers, and physicians.

Serve as mentor or preceptor for new staff and participate in formal training and process improvement initiatives.

Submit utilization/authorization requests to utilization management with documentation supporting the individual’s care plan.

Report critical incidents to appropriate internal and external parties such as state and county agencies (Adult Protective Services, Law Enforcement).

Assist and participate in appeals, grievances, and state audits.

Minimum Requirements

BA/BS degree and a minimum of 2 years of experience working with a social work agency, or equivalent education and experience.

Preferred Skills, Capabilities & Experiences

BA/BS degree in a health‑care related field.

Assisted living or social work experience.

Waiver experience.

Comfortable using technology.

Willingness to travel to worksite and other locations as necessary.
