Location & Schedule Home Care – VNA Erie County, Cheektowaga, NY Work Type: Full-Time Shift: 4:30 PM – 8:30 PM, Monday–Friday
Summary Coordinates the total Plan of Care for patients in the Home Care setting, assessing, planning, implementing, and evaluating. Provides leadership for the Health Care team by working cooperatively with team members to maintain standards of professional nursing practice.
Responsibilities
Coordinate the total Plan of Care for patients in the Home Care setting.
Assess, plan, implement, and evaluate patient care.
Provide leadership for the Health Care team, working cooperatively with other team members.
Qualifications
Associate’s Degree in Nursing.
Current RN license required.
1–5 years of relevant experience.
