Summary Coordinates the total Plan of Care for patients in the Home Care setting by assessing, planning, implementing, and evaluating. Provides leadership for the health care team by working cooperatively with other team members to maintain standards of professional nursing practice.
Responsibilities
Develop, implement, and evaluate care plans for patients in the home care setting.
Lead the healthcare team, ensuring collaboration and professional practice standards.
Assess patients’ medical status, devise care strategies, and adjust plans as needed.
Delegate tasks to LPNs and home‑health aides; prepare accurate, concise reports.
Communicate clearly with patients, families, and interdisciplinary staff.
Demonstrate flexibility, organizational skills, and sound judgment.
Education & Credentials • Associate’s degree in Nursing; graduate of an accredited School of Registered Nursing.
• RN licensure in New York State; valid NYS driver’s license.
Experience • 1–5 years of relevant experience.
• Minimum 1 year of medical/surgical RN experience and 1 year of infusion experience.
• Strong verbal and written communication skills to interact with patients, staff, and to prepare reports.
