Long Term Care facility in Bucks County, PA
is seeking an experienced
Registered Nurse Assessment Coordinator (RNAC)
to join its leadership team. This is an excellent opportunity for a detail-oriented RN who thrives in a structured, collaborative environment and is passionate about clinical accuracy, compliance, and resident-centered care.
This role offers a

The RNAC is responsible for coordinating and completing resident assessments—primarily the
Minimum Data Set (MDS) —to ensure accurate clinical documentation, regulatory compliance, and appropriate reimbursement. This role works closely with the interdisciplinary team to support high-quality care delivery and survey readiness.
Key Responsibilities
Coordinate and complete all required MDS assessments (admission, quarterly, annual, significant change, discharge)
Ensure assessments are accurate, timely, and compliant with CMS and state regulations
Collaborate with the Interdisciplinary Team (IDT) including nursing, therapy, social services, and dietary
Develop, update, and monitor individualized care plans
Oversee Medicare and insurance reimbursement accuracy (PDPM or applicable payment models)
Track assessment schedules and manage regulatory deadlines
Participate in care conferences with residents and families
Conduct clinical documentation audits and support survey readiness
Educate staff on documentation standards and assessment processes
Communicate changes in resident condition to providers and leadership
Compensation:
$100,000 - $104,000/yr (Depending on experience)
Schedule:
Full-time, Monday - Friday
Qualifications
Active Registered Nurse (RN) license (state-specific)
Prior experience in skilled nursing or long-term care preferred
Strong working knowledge of MDS, RAI process, and CMS regulations
Excellent clinical assessment, organizational, and communication skills
Proficiency with EHR/MDS software
High attention to detail with the ability to manage multiple deadlines
