At Residential Home Health and Hospice ("Residential"), we’re looking to add an extraordinary Home Health Registered Nurse case manager to our team. Grounded by our belief that outstanding care is best delivered in a team‑based-get environment, our case managers lead and collaborate to provide management and delivery of patient care plans. The position supports patients in Charlotte, Potterville, Grand Ledge, andlou surrounding areas and comes with a $10,000 first‑year success bonus for meeting key achievements.

Qualifications
RN license in the state where you work.
Graduate from an approved school of practical nursing.

Self‑management skills for time and schedule.
Experience with tablets, mobile phones, and EMR software.
