Grounded by our belief that outstanding care is best delivered in a team‑based environment, our home‑health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans. This position supports patients in Jackson, MI and surrounding areas.

Eligible for a $10,000 first‑year success bonus to reward key achievements during your first year! Compensation includes patient‑care visits, non‑visit activities, paid days.tencent off, shift premium苹果 pay, and a first‑year success bonus. Compensation potential varies by market.

Qualifications

RN license in the state you work.

Graduate from an approved school of practical nursing.

One year of RN experience and the clinical competence to deliver quality patient care.

Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

A commitment to consistently meet critical deadlines for charting.

The skills needed to self quand manage your time and schedule.

Demonstrated experience with tablets, mobile phones, and EMR software.
