Job Summary The Nurse Care Manager is key in collaborating with both internal and external treatment team members to offer education and support to clients at risk for or dealing with complex medical conditions. Join our compassionate, dedicated team, and make a meaningful impact on the lives of individuals in the Healthcare Home program. We're looking for a proactive problem solver who excels in building relationships and has a strong background in clinical practice. Your ability to work independently while contributing to a team will be invaluable as you help clients navigate their healthcare journeys.

You will focus on developing wellness initiatives, facilitating health education, and consulting with treatment teams to create tailored treatment plans for individuals with co-occurring chronic diseases.

Key Responsibilities

Collaborate with treatment team members to develop initial treatment plans for Healthcare Home clients.

Facilitate health education groups to enhance clients' understanding of complex medical conditions.

Implement wellness and prevention initiatives to promote healthier lifestyles.

Consult with patients on identified health conditions and provide guidance on their management.

Manage the admissions and discharges for hospital and medical provider clients within the Healthcare Home.

Train patients on their medical diagnoses, treatments, and medications effectively.

Utilize DMH Net Health technology programs and initiatives such as CyberAccess and Care Manager as required.

Monitor and report performance measures using advanced information systems tailored for nursing.

Education, Experience, and/or Credential Qualifications

Active LPN or RN license.

Experience with the IDD population preferred.

Additional Qualifications

Current driver’s license, acceptable driving record, and current auto insurance are required.

Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint checks.

Completion of New Hire Orientation and annual training requirements, including Relias.

Physical Requirements

Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body.

Repetitive movements of hands, fingers, and arms for typing and/or writing during work shifts.

Sedentary work involves sitting most of the time but may involve walking or standing for brief periods. Jobs are considered sedentary if walking and standing are required only occasionally.
